Are you referring to the guide on "Filling out CR-180: Petition for Dismissal"? If so, Step 7 says to attach a copy of CR-181, the order for dismissal, with the petition (CR-180) when you file with the court. Leave the order blank, except for the top "caption" portion, as the judge will check the appropriate box when he or she rules on the petition.
